South Dakota is requesting technical assistance with the transition from direct services to a coordinated care approach in the Children and Youth Special Healthcare Needs Program. South Dakota Health KiCC, the direct service reimbursement program, is being gradually phased out as the MCH program has put more focus on the DOH Health Home program, an extension of a successful SD Medicaid program that focuses on care coordination within primary care. Unfortunately, recruitment to this program has been slow. Potential clients who have opted out have cited a greater focus on specialty care, particularly for those high-need clients with multiple complex conditions. South Dakota is currently exploring potential partnerships for piloting/supporting care coordination in a medical specialty setting. Assistance is requested in developing and implementing a pilot utilizing best practices.
